Peer Evaluation Form (de Ramirez et al.)

 

The Peer Evaluation Form in the Appendix of de Ramírez et al. (1998, p. 16) prompts employees to list their peers and describe their effort on a scale from 0 to 3 where 3=Excellent job, 2=Did his/her share, 1=We had to force him/her to work, and 0=Did not work at all.
